Soft tissue management in the esthetic zone

Stefan Fickl

Soft tissue has gained interest in the field of implantology and periodontology. Today we know about the importance of a proper soft tissue mantle around dental implants to improve dental esthetics and long-term stability with regards to peri-implant infections.

Most of the techniques which are used around dental implants have been adopted from plastic periodontal surgery. Subepithelial connective tissue grafts are used to improve tissue volume around teeth and implants and free gingival grafts are used to improve the attached mucosa mainly around implants. In recent time xenograft replacement materials have been developed and are used to reduce morbidity in these surgeries. The goal of this presentation is to show different techniques for root coverage and soft tissue manipulation around teeth and implants.

Dr. Fickl received his dental degree from Erlangen University, Germany in 2003 and completed his postgraduate training in Periodontology and Implant Dentistry in Munich, Germany in 2007. From 2007 to 2009 he served as a Faculty Member at the Department of Peridontology and Implant Dentistry (Chair: Dr. Dennis Tarnow) at New York University, New York, United States. He then joined Würzburg University, Germany, where he received his PhD in 2011 and since then serves as an Associate Professor for Periodontology and Implant Dentistry. Dr. Fickl maintains at the same time a private office in Nuremberg-Fuerth, Germany dedicated to periodontics, implant dentistry and esthetics. Dr. Fickl is Fellow of the ITI, international speaker in the field of Periodontology and Implant Dentistry and author of numerous scientific publications in peer-reviewed journals and book chapters.

Workshop Title:

Prosthetic management of peri-implant tissues in the estehtic zone

Summary:

This hands-on will focus on the restorative management of the soft tissues around implant-supported restoration in the esthetic zone.

The participants will be exposed to the fabrication of implant supported provisional restorations getting familiar with immediate provisionalization procedures, peri-implant emergence profile and ovate pontic shapes.

Lecture:

Prosthetic management of peri-implant tissues in the esthetic zone. Digital versus analog protocol.

This presentation will focus on the restorative details that enhance the natural gingival architecture in esthetic implant-supported restorations. Morphologies of implant restorative profiles, materials and clinical procedures will be presented and discussed, following both analogue and digital work-flows.

Gianluca Paniz achieved his dental degree (DDS) at the University of Padova (Italy) in 2002.  He attended TUFTS University (Boston, USA) from 2003 until 2007 and achieved the Certificate of Advanced Education in Prosthodontics, the Master of Science (with a research on the esthetic outcome of different ceramic restorations) and the Certificate of Advanced Education in Esthetic Dentistry.

In 2016 achieved the title of Doctor Philosophiae (PhD) at the Univeristat International de Catalunja (Barcelona, Spain) with a research on the periodontal response to different prosthetic margin designs.

Gianluca Paniz is Adjunct Assistant Professor in the Department of Prosthodontics and Operative Dentistry at TUFTS University and Visiting Professor in the Department of Implantology at the University of Padova. Diplomate of the American Board of Prosthodontics and International Fellow of the American College of Prosthodontics, Member of the Italian Academy of Prosthetic Dentistry (AIOP), Active Member of the Italian Academy of Osseointegration (IAO) and of the of the Italian Academy of Aesthetic Dentistry (IAED).

Gianluca lectures nationally and internationally and is author or coauthor of several articles and book chapters.

Gianluca Paniz limits his practice to prosthodontics in his office in Padova, Italy.

Title : How to deal with Sinus Grafting Complications : from prevention to treatment.

Objectives:

– Analyze the anatomy of the maxillary sinus by identifying the various risk factors that could compromise the success of sinus bone grafts.

– To know how to manage and prevent the per and post-operative complications of sinus grafts, based on a clinical and radiological analysis.

– Address the latest advances and technological innovations related to sinus lifts to avoid complications.

Abstract:

The maxillary sinus graft is nowadays performed in about 50% of atrophic posterior maxilla for a proper implant rehabilitation. Various approaches have been described to access the maxillary sinus by delicately detaching the sinus mucosa. During this presentation, we will review the surgical anatomy and highlight, through a preoperative radiological analysis, the various anatomical risk factors, and manage the various complications that could compromise the success of sinus grafts.

The latest advances and technological innovations related to sinus lifts will be exposed as piezo-electric surgery; The management and prevention of complications will be discussed in detail, based on clinical and radiographic analysis. In short, the preoperative assessment as well as the per and postoperative complications of sinus grafts will be debated under both preventive and therapeutic aspects.

Immediate implant placement in the esthetic zone

Nowadays tooth replacement in the aesthetic zone is a valuable treatment option with many advantages and benefits. In the same time, when we have patients with medium or high smile line and high esthetic demands, it can be very challenging.  Fine relationship between the restoration and the gingiva needs to be established. This brings the focus on the transitional zone. Different surgical and prosthetic tricks come into play. The lecture will focus on the key details that I believe will make the difference.
